We need a php/mysql application within insurance handling and registration of damages.

There needs to be company accounts (customers, repair garages and insurance companies) with their own users accounts, one client company/insurance company can have one or several user accounts.

The application needs to have support for english and swedish language (the translation to swedish can be done by me) and it needs a template system so the style/design can easily be changed. The quality of design is not important, since the design can be improved changed later, but it's important with jquery or ajax/javascript to make the registration progress easy and nice looking.

Registered users (that the companies have created in the program) then needs to be able to register a damage/insurance case, in like 4 steps giving around 30-40 fields of info, some text fields and some select options, including pointing to a picture of the car to show were the damage is.

All registered cases of the customer laters need to be on a list for the customer, with option to update missing info. A complete case then needs to approved manually of a responsable operator of the program.

After that several cases during one month are all complete and approved, an invoice needs to be created in pdf-format to be sent by email to the insurance company with like 20 or more finished cases. Not automaticly, but manually done by the program operator/s.

Some reports taken out from the cases later needs to be produced and possible to send via email, perhaps in xls-format. Since all cases will have an estimated cost for repair, reports of the sum of costs and cases for one customer or simular reports needs to be possible.
