1. Design a website that allows Emergency Room Physicians to Book Appointments and tests for patients when they are discharged.

2. Physicians should be able to choose how fast patient needs follow up eg Next Day, within a week

3. What Kind of follow up the patient Needs, General Cardiology, heart failure, Electrophysiology or Interventional Cardiology

4. A calendar should pop up which will allow the physician to see available slots for appointments.

5. After selecting an appointment they would be redirected to a form

6. The ER physician would enter several fields in a Form
a. Name
b. Patient Phone:
c. Insurance (drop down menu)
d. Reason for consult (drop down menu)
e. Any tests Needed (drop down menu eg Stress, Echo, Cath MRI)
f. A large window to cut and paste any Notes the physician has on the patient,
g. Emergency Physicians name.

7. A one page letter should be generated that the ED physician can give to the patient with all the appointment details.
