I'm after someone who can create a very simple iphone app

Here's my requirement:
On Iphone and Andriod it's only one page
- Buttonto a website
- Locate us button - open up another page with address and maps and some text on how to get there
- Link to VDO page - This page will pull the vdo that was uploaded by 'username' from youtube (like subscribed person)
once click on the 'vdo' then it's play

That's it for now, can you please quote:
- how much it will cost (as it will be a few apps not just once off)
- how long it will take to do
- your previous portfolio or works
