I have a small PHP application that I need built.  The database is already done.  What is needed is a form, that is powered by AJAX, that allows a ticket to be submitted and then to display the current status of the job as well as the history of the comments of the ticket posted.

The user credentials will come from a centralized access server, accessible through LDAP.

The task:
1.  Create a basic design to house the application
2.  Have an administration panel to be able to add service users (only administration needed)
3.  Create a form that will be used by both the service department and the submitter of the ticket.  Display history of who worked on the ticket and any history/comments on the ticket screen.
4.  Allow users to login via LDAP (just setup to go to a LDAP server and get authentication)

The entire project, I estimate willl take a total of 20 hours or less.
