Need a database-driven website to be developed from scratch in PHP. OOP is a must, to ensure scalability. Needs a backend with user-groups for different access/security settings. Functionality within the website must also include a custom form/field generator and the ability to create reports. Again, scalability is important. 
Will need web design once the database is created. I have a template that I would like to use; however, modifications need to be made to accommodate banner advertisements.
