I'm looking for someone to code a Paypal payment form to be integrated into a wordpress site based on the Thematic framework.

The form must - before allowing the user to checkout-  take basic details such as ticket holder's name, address, email, etc. and submit them to a mySQL database.

Please look at the attched pdf for a guide as to what I need.

The form only deals with the section 'booking tickets for individuals'.
