There are not too many design requirements of this project.

If you accept the project we would require a detailed functional requirements to make sure the requirements are clearly understood and properly implemented.

A portion of this project also requires the server module to be working. In other words, it has to connect to the server. If any code that is required on the server would be expected to be part of the deliverable. This includes authentication. Before the iPhone app can download the questionnaire from the server it needs to be authenticated.
